This role involves functioning as a provider within the interdisciplinary team, focusing on the direct provision of home-based medical services to a designated patient population. The position emphasizes improving patient health and well-being through collaboration with primary care providers, interdisciplinary team members, and other healthcare professionals to establish and implement personalized care plans throughout the patient journey.

Key Responsibilities:

  • Adhere to state regulations governing advanced practice nursing.
  • Understand and comply with policies related to long-term care and assisted living.
  • Build and maintain effective relationships with patients, their representatives, facility staff, and collaborating healthcare providers.
  • Conduct comprehensive assessments for new patients and update care plans as necessary.
  • Perform follow-up assessments post-hospitalization in accordance with established procedures.
  • Prescribe appropriate diagnostics and interventions to minimize unnecessary hospital admissions.
  • Consult with hospital physicians regarding patient transfers.
  • Collaborate with utilization management staff and primary care providers to determine medical necessity for services.
  • Educate patients and their representatives about disease management and wellness promotion.
  • Provide support and training to facility staff.
  • Engage in management meetings, including case reviews and quality improvement initiatives.
  • Participate in continuing education and professional development activities.
  • Remain on-call for assigned caseload during business hours and participate in clinical on-call programs as needed.
  • Stay informed on relevant medical research and advancements in nursing practice.
  • Perform additional duties as assigned.

Required Qualifications:

  • Completion of an accredited Nurse Practitioner or Physician Assistant program.
  • Current RN license if functioning as a Nurse Practitioner.
  • Active NP or PA license.
  • Certification from recognized professional bodies such as AANP, ANCC, or NCCPA.
  • Familiarity with long-term care systems and community resources.
  • Access to reliable transportation and compliance with safety policies.

Preferred Qualifications:

  • Minimum of 2 years of clinical experience.
  • Knowledge of coding systems including CPT, ICD-9, and HCPC.
  • Understanding of clinical care standards.
  • Awareness of NCQA requirements and CMS guidelines.
