Job Overview for Rehabilitation Nurse:

As a Rehabilitation Nurse, you will play a vital role in assisting patients with disabilities and chronic conditions to regain and maintain their highest level of functionality. Your expertise will help clients transition to new lifestyles while fostering a supportive and therapeutic environment for their growth.

Key Responsibilities:
  • Timely completion of referrals and acquisition of verbal orders from physicians.
  • Conduct thorough physical assessments, identify patient needs, and collaborate with healthcare providers to formulate effective care plans.
  • Assess the necessity for additional health services and ensure appropriate referrals are made.
  • Evaluate the home and family environment to enhance patient care and safety.
  • Engage patients and their families in the development of personalized care plans with achievable objectives.
  • Coordinate with a multidisciplinary care team to strategize patient care and maintain accurate documentation.
  • Provide clear guidance to Home Health Aides and Licensed Practical Nurses (LPNs).
  • Administer skilled nursing care in accordance with established care plans, making adjustments as necessary.
  • Regularly update physicians regarding patient progress and care developments.
  • Initiate nursing procedures for patient care and offer essential instructions to caregivers.
  • Communicate effectively with patients and families about diagnoses, treatment options, and modifications to care plans.
  • Deliver specialized care and continuously enhance professional competencies.
Qualifications:
  • Graduation from an accredited nursing program.
  • Possession of a current and valid nursing license.
  • A minimum of one year of experience in rehabilitation or skilled nursing settings.
  • Current certifications in AHA BLS and ACLS, if applicable.
