Home Health Aide - Community Based Care

PRN AS NEEDED (Days/Times Vary)

As a Home Health Aide with Community Based Care, you will be instrumental in improving the quality of life for our clients by assisting them with daily living tasks and ensuring their overall comfort and safety. You will collaborate closely with our healthcare professionals to provide individualized care that meets the specific needs of each client.

Responsibilities

  • Provide personal care services as required.
  • Deliver personal care under the guidance and supervision of a Registered Nurse.
  • Follow established care plans effectively.
  • Assist with meal preparation and feeding.
  • Perform laundry duties.
  • Conduct light housekeeping tasks.
  • Adhere to infection control protocols, including proper hand hygiene.
  • Report any changes in the client's condition or needs to the supervising Registered Nurse.
  • Maintain accurate documentation of care provided and complete necessary forms for client records.
  • Ensure personal safety and a secure environment for clients by following infection control practices and reporting any unsafe conditions to the Supervisor/Case Manager.
  • Demonstrate safe usage of equipment, ensuring orientation is completed before use.
  • Communicate effectively with all members of the healthcare team through verbal updates and participation in meetings.
  • Treat clients and their families with dignity and respect, maintaining confidentiality at all times.
  • Accept and fulfill assigned duties responsibly, exercising sound judgment.
  • Participate in mandatory training programs to meet compliance standards and identify personal learning needs.
  • Perform additional related duties as required.

Qualifications

  • Meet the training and/or certification standards set by the state for home health aide services.
  • Hold an active and verifiable State Tested Nursing Assistant (STNA) or Certified Nursing Assistant (CNA) certification, or possess 1 year of supervised experience within the last 3 years.
  • Be at least eighteen (18) years old.
  • Exhibit the ability to read, write, and follow a written Plan of Care.
  • Demonstrate strong interpersonal skills and a genuine passion for caregiving.
  • Possess a valid driver's license and maintain a good driving record.
  • Previous experience of 1+ years is highly preferred.
  • Ability to frequently stand, walk, bend, and occasionally squat or kneel.
  • Capability to handle weights as required.
