Team Composition: 12 Physicians, 4 Advanced Practice Providers, and 2 Medical Assistants

Required Procedures: Office consultations, Disability Evaluations, and Botulinum Toxin Administration (Botox)

Technology: EMR system utilized is Epic

Compensation Package:
Travel, accommodation, and malpractice insurance are included.

Qualifications:
- Active Ohio Medical License
- Board Certified
- Basic Life Support (BLS) Certification
- Clean professional record
- Full-time availability required for Monday to Friday, 8 AM to 4 PM
- Proof of COVID Vaccination is mandatory.
