About the Role:We are looking for a dedicated and compassionate Physical Therapist to provide high-quality physical therapy services to patients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patients' therapeutic, rehabilitative, and functional status, and developing a personalized plan of care.
Responsibilities:- Provides physical therapy services to patients according to a written physician's plan of care.
- Evaluates home environment and assesses for muscle strength, mobility, gait, and ROM potential for rehab.
- Directs physical therapy treatment and instructs patients and families/caregivers in the use and care of therapeutic appliances.
- Determines priority needs for physical therapy and reports to physician patient's reaction to treatment or changes in condition.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy.
- Possesses a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
- Licensed to practice as a physical therapist within the State.
- Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
- Demonstrates good verbal and written communication and organization skills.
- Possess and maintains current BLS Hands On CPR Certification.
