Care Coordinator Specialist

We are seeking a highly organized and detail-oriented Clinical Care Coordinator to join our team at CooperRiis. As a key member of our Integrative Clinical Recovery Team, you will play a vital role in supporting residents in their recovery process.
Key Responsibilities- Collaborate with the Clinical Recovery Team to develop and implement recovery plans
- Work closely with the team to promote residents' movement through the program and address logistical barriers
- Support residents in their transition process between levels of the program and/or discharge process
- Develop and maintain a library of resources for referral purposes
- Communicate and coordinate special requests, transition planning, and other logistics processes
- Provide crisis support and work closely with the Coaching Team and Office Manager
- Bachelor's Degree preferred or equivalent relevant experience
- Experience working with adults recovering from significant mental health challenges, preferably in a residential mental health or substance abuse setting
- Strong organization, time management, and administrative skills
- Competitive hourly rate starting at $18.50 and increases based on experience
