About the Job

YesCare Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the assessment of patients' health status.

Responsibilities
  • Contribute to the assessment of patients' health status
  • Participate in the development and modification of patients' plans of care
  • Perform delegated tasks, activities, and functions consistent with prior educational preparation and within scope of practice
  • Provide direct basic care to assigned multiple clients within the structure of the healthcare program at their facility
  • Provide safe and effective nursing care and assume responsibility for the care delivered
  • Implement patient education based on established teaching plans
  • Utilize a problem-solving approach as the basis for decision-making
  • Monitor offsite and onsite scheduling with a collaborative approach
  • Monitor consult process as it relates to policy and contract
  • Participate in Quality Improvement
  • Participate in Patient Care Committee Meeting
  • Monitor CARES- with coordination as needed
  • Ensure onsite and offsite follow-up care of patients along with telemedicine appointments
  • Work in collaboration with Regional Medical Director, site medical providers, Regional Nursing Director, and other site nursing leaders
  • Perform other nursing duties as assigned
