Key Responsibilities:

  • Administer medications and perform necessary treatments.
  • Assist in the admission, transfer, and discharge processes of residents.
  • Document care services accurately in resident medical records and departmental logs.
  • Provide education to residents and their families regarding treatment plans, rehabilitation, and changes in health status.
  • Adhere to infection control protocols and all facility policies.
  • Consult with physicians regarding residents' care plans and report any significant changes.
  • Ensure the confidentiality and privacy of resident information.
  • Maintain high standards of care in compliance with DOH regulations and facility guidelines.
  • Collaborate with the interdisciplinary team to address and resolve resident concerns.

Qualifications:

  • Must have a nursing degree from an accredited institution or be a graduate of an approved LPN program.
  • Preferred experience in long-term care settings.
  • Strong communication skills, a positive demeanor, and a collaborative spirit.

Shift Availability:

All shifts are open.

Compensation:

$32.00 per hour for Day shifts, $34.00 per hour for Evening/Night shifts, commensurate with experience.
