Occupational Therapist Opportunity

Sun West Choice Healthcare & Rehabilitation in Sun City West, AZ, is seeking a Full Time Occupational Therapist to join our in-house therapy team.

Key Responsibilities:
  • Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders and develops appropriate plans of care following all regulatory and clinical practice standards.
  • Provides rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices.
  • Evaluates treatment outcomes for effectiveness and modifies services in relation to patient evaluation and referrals.
  • Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
  • Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements.
