Nurse Care Coordinator

Responsibilities:
- Coordinate with nursing staff and support personnel to plan and implement resident care plans.
- Review and update resident care plans to ensure they meet the individual needs of each resident.
- Collaborate with interdisciplinary teams to ensure seamless care transitions and effective communication.
- Provide leadership and guidance to nursing personnel to ensure high-quality care is delivered.
Qualifications:
- Current, unencumbered LPN license in the state of Oregon.
- Experience in long-term care or a related field.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
