Job Summary:The Registered Nurse will be responsible for delivering comprehensive end-of-life care to patients and their families, working closely with our interdisciplinary team to ensure seamless care coordination.
Responsibilities:- Provide direct and indirect skilled nursing care to patients in their homes, including administering medications, wound care, and vital sign monitoring.
- Develop and implement individualized care plans, working closely with patients, families, and healthcare teams to ensure patient-centered care.
- Conduct thorough assessments, identify patient needs, and develop strategies to address those needs.
- Collaborate with other healthcare professionals to ensure continuity of care and effective communication.
- Document patient progress, including vital signs, medications, and treatment plans.
- Current RN licensure in the State of New Jersey.
- At least 1 year of experience in hospice, home care, or community health.
- Ability to work independently with minimal supervision and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive patient information.
