About the Role:- Develop and implement discharge plans for patients requiring post-acute care or follow-up services.
- Collaborate with healthcare teams, patients, and families to identify and address patient needs.
- Utilize clinical judgment and expertise to coordinate care and ensure optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange.
- Participate in quality and risk management initiatives to improve patient care and outcomes.
- Graduate of an accredited school of nursing with a BSN preferred.
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts with a minimum of three years' clinical experience.
- Evidence of continued education and professional development, with CCM or ACMA certification preferred.
