We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Case Manager. As a key member of our interdisciplinary team, you will provide exceptional care to patients and their families at the end of life.

As a Registered Nurse with Magnolia Health Systems, you will have the opportunity to work in a variety of settings, including hospice care, home care, and skilled nursing facilities. You will be responsible for assessing patient needs, developing and implementing individualized plans of care, and coordinating with other healthcare professionals to ensure seamless transitions.

About the Role:

  • Provide professional nursing care to patients and families in hospice and long-term care settings.
  • Assess patient needs and develop individualized plans of care in collaboration with the interdisciplinary team.
  • Coordinate with other healthcare professionals to ensure seamless transitions and continuity of care.
  • Document all interactions and interventions in accordance with agency policies and procedures.
  • Participate in education and training programs to stay current on best practices and industry developments.

Requirements:

  • Bachelor's degree in nursing preferred.
  • Minimum of one year clinical nursing experience; hospice experience preferred.
  • Demonstrated knowledge of regulatory requirements for hospice care.

Educational Background:

  • Registered Nurse with current license to practice in Indiana.

Work Environment:

  • Travel between long-term care facilities and/or place of primary residence.

Salary Range: $65,000 - $85,000 per annum.
