Key Responsibilities- Deliver high-quality patient care, including assistance with daily living activities, medication reminders, and vital sign monitoring.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Maintain accurate and detailed records of patient interactions and care delivered.
- Participate in ongoing education and training to enhance clinical knowledge and skills.
- CNA certification in good standing.
- High school diploma or equivalent required; associate's or bachelor's degree preferred.
- At least six months of experience in providing personal care services, preferably in a hospice or home health setting.
- Excellent communication and interpersonal skills, with ability to work effectively in a team environment.
- Valid driver's license and reliable transportation.
