Position Overview:
The primary responsibility of the Licensed Vocational Nurse (LVN) is to deliver essential nursing care to patients facing common health issues. The LVN collaborates as part of a multidisciplinary healthcare team, employing the nursing process to implement patient care under the guidance of a registered nurse. All responsibilities are carried out in alignment with regulatory standards, organizational policies, and performance improvement protocols.
Key Responsibilities:
- Provide high-quality nursing care to a designated group of patients by gathering data and recognizing deviations from the norm; assist in reassessing patients based on assignments and unit protocols under the supervision of a registered nurse.
- Thoroughly document all findings in patient records and identify expected outcomes based on assessment data; effectively communicate significant assessment results with the healthcare team.
- Execute nursing interventions outlined in the individualized multidisciplinary treatment plan, adhering to accepted standards of practice in collaboration with other healthcare professionals; perform nursing procedures as directed by physicians.
- Conduct specified bedside laboratory tests, manage wound care, and assist patients with activities of daily living (ADLs).
- Support patients in coping with and adapting to their health conditions and associated stressors.
- Deliver age-appropriate communication and culturally sensitive interventions, documenting any special needs.
- Ensure the safe administration of medications in accordance with facility policies and physician orders; assess patients before and after medication administration, accurately documenting doses and providing education on medications.
- Follow up on patient care tasks; communicate any incomplete assignments and patient needs with other disciplines to ensure continuity of care; accurately transcribe orders and perform chart audits as per facility guidelines.
- Utilize nursing knowledge to effectively meet the needs of the facility, patient population, and staff; leverage nursing resources to enhance evidence-based practices.
- Guide, educate, and mentor new staff as a preceptor; undertake additional duties and projects as assigned.
Current, valid, and active license to practice as a Licensed Vocational/Practical Nurse in the state of employment. Valid Compact licenses are also acceptable. A High School Diploma or GED is required. A minimum of one year of inpatient hospital experience is strongly preferred. The LVN must possess effective communication skills, both verbal and written, and demonstrate organizational abilities. Understanding and sensitivity to serve a culturally diverse and special needs population is essential. Critical thinking skills are necessary for the continuous improvement of patient care. The ability to maintain composure in high-stress situations is crucial. Successful completion of CPR certification and an approved behavioral health de-escalation program is required. This role may involve exposure to various environmental factors and requires spending a significant amount of time indoors.
