Job Summary

LHC Group is seeking a highly skilled Full-Time Home Health Therapist to join our team in North Chelmsford, Massachusetts.

About the Role

We are looking for a compassionate and dedicated Physical Therapist to provide high-quality care to patients in their homes. As a member of our team, you will be responsible for assessing patient needs, developing treatment plans, and delivering evidence-based interventions to improve functional status and overall well-being.

This is a full-time position with a competitive salary range of $120,000 - $150,000 per year, commensurate with experience. Our benefits package includes medical, dental, and vision coverage, as well as a 401(k) retirement plan and opportunities for continuing education and professional growth.

Responsibilities
  • Evaluate patient functional status and physical therapy needs
  • Develop and implement treatment plans to achieve patient goals
  • Deliver evidence-based interventions to improve patient outcomes
  • Collaborate with multidisciplinary teams to coordinate patient care
Requirements
  • Current Physical Therapy licensure in the state of Massachusetts
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
