Schedule and Benefits- Full-Time: 42 hours per week
- Rotating Schedule: 7:00 AM – 9:00 PM

Key Responsibilities- Deliver high-quality patient care in accordance with our standards and regulations.
- Develop and implement individualized patient plans to meet their unique needs.
- Collaborate with healthcare teams to ensure seamless care coordination.
- Monitor patient outcomes and make adjustments as necessary.
- Document all patient interactions and care provided in our electronic health records system.
- Licensed Practical Nurse / LPN license in good standing.
- Previous private duty, home health, hospice, hospital or skilled nursing experience preferred.
- Active CPR certification.
- Familiarity with electronic health records.
- Strong problem solving and critical thinking skills.
- Reliable transportation, valid driver's license and current automobile insurance.
