Schedule and Benefits
  • Full-Time: 42 hours per week
  • Rotating Schedule: 7:00 AM – 9:00 PM

Key Responsibilities
  1. Deliver high-quality patient care in accordance with our standards and regulations.
  2. Develop and implement individualized patient plans to meet their unique needs.
  3. Collaborate with healthcare teams to ensure seamless care coordination.
  4. Monitor patient outcomes and make adjustments as necessary.
  5. Document all patient interactions and care provided in our electronic health records system.
Requirements
  • Licensed Practical Nurse / LPN license in good standing.
  • Previous private duty, home health, hospice, hospital or skilled nursing experience preferred.
  • Active CPR certification.
  • Familiarity with electronic health records.
  • Strong problem solving and critical thinking skills.
  • Reliable transportation, valid driver's license and current automobile insurance.
