About the RoleThis is a full-time position in our Health Hub, where you will work collaboratively with our team to deliver comprehensive, evidence-based care to patients of all ages. You will have the autonomy to run your own practice, with the support of our clinical care team and access to the latest tools and resources.
Responsibilities- Provide primary and family care to patients, including accurate assessment, diagnosis, treatment, and management of health problems.
- Perform routine examinations, health screenings, and diagnostic tests to help diagnose and treat patients for injuries or illnesses.
- Coordinate with other healthcare professionals to maintain a comprehensive plan of care and ensure positive patient health outcomes.
- Encourage patients to become active participants in their own healthcare and well-being through services such as health education, behavioral health, and wellness programs.
- Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role.
- Minimum of two years of medically relevant experience or equivalent.
- Basic Life Support (BLS) certification, including performing CPR and operating an Automated External Defibrillator (AED).
- Ability to speak and write in additional languages, such as Spanish, depending on the market.
