About the Role

Wexford Health Sources, a leading correctional healthcare provider, is seeking a dedicated Licensed Practical Nurse (LPN) to join our team.

Job Summary

The LPN will play a vital role in delivering high-quality patient care through assessment, planning, implementation, and evaluation. Under the clinical guidance of Registered Nurses, the LPN will direct and guide patient teaching activities, ensuring seamless care coordination.

Responsibilities
  • Assist in the assessment of patients' physical, psychological, and social dimensions.
  • Develop and implement individualized treatment plans in collaboration with healthcare teams.
  • Administer medications and interventions as directed by healthcare providers.
  • Document patient encounters, response, and outcomes, communicating effectively with supervisory personnel and physicians.
  • Maintain accurate medical records and ensure confidentiality.
  • Serve as a member of the nursing care and treatment team, assisting physicians as assigned.
Requirements

To be eligible for this role, you must possess:

  • A current, unrestricted AZ State Licensed Practical Nursing (LPN) License.
  • Current CPR Certification.
  • Graduation from an accredited school of nursing.
  • A minimum of one year of clinical experience; correctional experience preferred.

Salary Range

$45,000 - $65,000 per annum, depending on experience.
