About the Role

As a skilled Nurse Practitioner with Abode Care Partners, you will be responsible for delivering high-quality medical care to seniors and individuals with complex needs in various settings. Your primary focus will be on examining and treating chronic and urgent episodic illnesses, minor injuries, and other health issues.

To excel in this role, you must possess strong interpersonal communication skills, both verbal and written. You should also be proficient in using electronic medical record (EMR) systems, medical instruments, and equipment.

In addition to your clinical expertise, you will collaborate with caregivers and other healthcare professionals to provide interdisciplinary care and maintain comprehensive patient plans. This involves ensuring patient medical records are accurate and up-to-date.

Responsibilities

\
  • Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other health issues.
  • Order and interpret laboratory and diagnostic testing as needed.
  • Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining comprehensive patient plans.
  • Provide referrals to other healthcare practitioners when necessary.
  • Ensure patient medical records are accurate and up-to-date.

Requirements

\
  • Active state Nurse Practitioners license.
  • Current Nurse Practitioner National Board Certification.
  • Active state DEA licensure.
  • Master's degree in Nursing (MSN).
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Strong interpersonal communication skills – verbal and written.
  • Excellent time management skills with accountability for practitioner's daily workflow.
  • Proficiency in medical instruments and equipment.
  • Strong knowledge base of HIPAA and OSHA regulations.
  • Demonstrated proficiency in computer-based data management programs, EMR systems, and Microsoft Office applications.
