Job Summary:

As an Occupational Therapist with Mercy Medical Center Home Health and Hospice, you will play a vital role in providing high-quality patient care in the comfort of their own homes. Our team is dedicated to delivering compassionate and personalized care to patients with a range of health conditions, and we are seeking a skilled and empathetic Occupational Therapist to join our team.

Key Responsibilities:

Provide skilled occupational therapy services to patients in their homes, as ordered by the attending physician
Develop and implement individualized plans of care to help patients achieve their goals and improve their functional abilities
Conduct thorough evaluations of patients' physical and cognitive abilities, and develop strategies to address any deficits or limitations
Educate patients and their families on proper use of equipment and techniques to promote independence and safety
Collaborate with other healthcare professionals to ensure comprehensive and coordinated care

Requirements:

Current license to practice as an Occupational Therapist in the state of employment
Minimum one year of experience as an Occupational Therapist in an acute care setting, with home care experience preferred
Valid driver's license and auto insurance
Ability to lift, bend, and move patients as needed
