Job Summary

We are seeking a skilled Community Based Nurse to join our team. This role will be responsible for providing comprehensive healthcare supports, training, and technical assistance to individuals diagnosed with I/DD. The successful candidate will work collaboratively with agency nurses, healthcare coordinators, and other stakeholders to ensure the highest quality of care.

Key Responsibilities:
  • Provide clinical consultation and resource guidance on medical treatment and care to individuals with complex health conditions.
  • Collaborate with agency nurses to address healthcare issues and provide support to individuals and their teams.
  • Establish alliances with other nurses and healthcare professionals working in the community with individuals who have been diagnosed with I/DD.
  • Develop and deliver training programs on physical and behavioral healthcare issues for staff members, family members, and individuals with I/DD.
  • Provide technical assistance on PCHC initiatives, such as Behavioral Health Team Reviews and Health Promotion Activity Plans.

Requirements:
  • Registered Nurse with a valid Pennsylvania Nursing License.
  • At least two years of experience working with individuals with I/DD or in public health nursing, community health nursing, or behavioral health nursing.
  • Excellent communication and interpersonal skills.
