Job Title: Diagnostic Treatment Center Nurse

Mitchell Martin is seeking a skilled Diagnostic Treatment Center Nurse to join our team. As a Diagnostic Treatment Center Nurse, you will be responsible for ensuring the quality and safe delivery of nursing services in the Diagnostic & Treatment Center (DTC) and Day Health Center (DHC).

Key Responsibilities:
  • Assess, plan, implement, and coordinate nursing care in the Medical Practice and DHC.
  • Coordinate and collaborate with the Site Medical Director and members of the interdisciplinary team (IDT) regarding Infection Control and Surveillance activities at the PACE Center.
  • Administer medication and treatment as prescribed by the PCP.
  • Investigate and assist in documenting and resolving customer issues and grievances in a timely, accurate, and courteous manner.
  • Provide prescribed or emergency care to patients requiring professional nursing services, including: venipuncture, dressing changes, straight catheterization, suctioning, immunizations, etc.
  • Perform diagnostic tests as authorized by the Primary Care provider (PCP). Obtain laboratory specimens when indicated.
  • Provide treatment under medical direction.
  • Assess, evaluate, and report to physician the participant's physical and emotional condition, reaction to medications and/or treatments, and/or critical laboratory result requiring PCP intervention.
  • Independently perform skilled nursing procedures, assessments, and techniques based on specific Participant's needs while at the PACE Center.
Requirements:
  • Current active and unrestricted license and registration in NYS as a Registered Nurse, required.
  • Minimum of one (1) year general clinical experience in a certified home health agency (CHHA), acute care, medical surgical, and/or critical care, Nursing Home experience required.
  • Customer Service experience required.
  • Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Additional Requirements:
  • Be legally authorized (for example, currently licensed, registered, or certified if applicable) to practice in the State in which the healthcare professional will perform the function.
  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Current Driver's License, required insurance, and car available for work as required.
  • Is self-directed with ability to work with or without supervision.
  • Proficient in use of a computer for email, word processing, research, and maintenance of Participant records.
  • Demonstrated ability to establish good rapport with participants and peers is essential.
  • Tactful and courteous.
  • Pleasant telephone manner.
  • Must be able to work weekends.
  • Excellent written and verbal communication.
  • Professional attitude.
