About the Role:
- Assess, diagnose, and manage health/illness concerns for clients/residents in various settings.
- Perform advanced comprehensive and focused health assessments, ordering investigative procedures as needed.
- Communicate effectively with clients/residents, discussing health findings, outcomes, and prognosis.
- Continuously monitor client response to treatment plans and make adjustments as necessary.
Responsibilities:
Assessment, Diagnosis, and Treatment- Achieve optimal physical and emotional functioning through long-term approaches to client care.
- Apply principles of harm reduction and collaborate with clients/residents to develop individualized plans.
- Encourage maximum independence and accountability for self-care.
- Document clinical care in accordance with Prairie Mountain Health documentation practices and policies.
- Establish relationships with family physicians and healthcare teams to ensure coordinated services.
- Maintain expertise as a clinical resource for client/resident care delivery.
- Demonstrate skill in formal and informal consultation and collaboration with all disciplines.
- Model client-centered care and promote collaborative practice among team members.
- Select and prescribe drugs based on principles of pharmacology and treatment plans.
- Monitor client response to drug therapy and make necessary changes.
- Educate clients/residents on recommended drug use and provide support.
