Job Summary

We are seeking a highly skilled Registered Nurse to join our Home Care/Hospice team. As a RN Case Manager, you will be responsible for a caseload of patients from admission to discharge. Your primary focus will be on providing high-quality patient care, developing and modifying care plans, and coordinating with interdisciplinary teams to ensure seamless care transitions.

Key Responsibilities

• Admission and revisit visits to assess patient needs and develop care plans
• Coordination of after-hours and overflow visits to ensure continuous care
• Development and modification of patient care plans to meet individual needs
• Coordination of ancillary services, including rehabilitation therapists, social workers, and home health aides
• Completion of required documentation, including discharge summaries
• Monitoring of fiscal performance and OASIS data collection
• Performs IV Therapy visits and rotates on-call
• Performs field supervision of LPNs and aides

Requirements

• High School Diploma or equivalent required
• Graduate of an accredited school of nursing required
• Licensed to practice as an RN in Ohio required; licensed to practice as an RN in Kentucky to be obtained after hire
• BMVL required; must maintain eligibility to drive as determined by SOMC insurance carriers
• BSN preferred; BLS within 90 days of hire required
• One year prior nursing experience in the acute or long term care setting required
