Key Responsibilities- Develop and implement individualized care plans to address substance use and behavioral health needs.
- Conduct in-home assessments, therapeutic clinical interventions, and physical assessments/triage to ensure comprehensive care.
- Collaborate with our interdisciplinary team to develop, implement, and evaluate patient care through regular team huddles.
- Document patient care with accurate and accessible records, maintaining confidentiality and adhering to regulatory requirements.
- Participate in our Primary Care Quality Assurance program, assisting with quality audits and identifying opportunities for improving care.
- Distribute medication to patients and maintain accurate records for narcotics and controlled substances in accordance with law and agency policies.
- Valid New York State Licensed/Registered Nurse.
- Minimum of 5 years of experience in a Behavioral Health/Psychiatric outpatient/hospital setting supporting mobile patients with Severe Psychiatric Disorders (SMI).
- Familiarity/experience with medications related to various Psychiatric/Behavioral Health Disorders.
- Experience administering injections and collaborating on treatment planning.
- Experience working with culturally diverse community populations.
