Position Overview:
The Nurse Practitioner plays a crucial role in delivering both formal and informal patient care, while also engaging in educational initiatives and contributing to the formulation and enhancement of specific policies, procedures, and clinical guidelines.
Key Responsibilities:
- Conduct comprehensive health assessments to diagnose and treat patients effectively.
- Collaborate with physicians to provide patient and family counseling regarding diagnoses, testing, and procedural risks.
- Prescribe both pharmacological and non-pharmacological treatments within the established scope of practice.
- Facilitate referrals to other healthcare professionals and community resources as necessary.
- Educate patients and their families on disease management, treatment options, and post-discharge care.
- Act as a liaison between nursing staff and physicians to ensure seamless communication and care delivery.
- Complete necessary documentation, including insurance and home health forms.
- Maintain educational materials and provide training on medical devices and equipment.
Qualifications:
Current certification as an Adult Nurse Practitioner in Virginia is required. A minimum of five years of recent experience in adult medical-surgical nursing is preferred.
