Physical Therapist Home Health PRN Specialist

We are seeking a highly skilled and compassionate Physical Therapist to join our team at Pinnacle Home Care Inc. As a Physical Therapist, you will play a vital role in delivering exceptional care to our patients in the comfort of their own homes.
Key Responsibilities:- Conduct initial assessments of patients to determine home care needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establish and implement treatment plans in consultation with physicians and revise as necessary.
- Teach, supervise, and counsel patients and their families in physical therapy programs.
- Communicate with physicians and other healthcare professionals to ensure seamless care coordination.
- Supervise Physical Therapist Assistants and provide guidance and support as needed.
- Graduate of an accredited Physical Therapy program or equivalent education.
- Licensed to practice as a PT in the State of Florida.
- Current CPR Certification.
- Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
- At least one year of experience as a Physical Therapist, with home healthcare experience preferred.
