As an Advanced Practice Clinician, you will have the opportunity to conduct in-home assessments and provide evidence-based practice screenings to patients in the comfort of their own homes. Your primary responsibilities will include evaluating past medical history, reviewing systems, medication reconciliation, vital signs, and comprehensive physical exams.
You will also be responsible for performing point-of-care testing, identifying diagnoses, formulating lists of current and past medical conditions, and communicating findings with members' PCPs. In addition, you will educate members on disease processes, medications, and compliance.
This role requires travel across Washington and Oregon, approximately 85% of the time, with all travel expenses covered. You will receive a base salary plus multiple incentives.
Key Responsibilities:
- Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings, including point-of-care testing (as appropriate)
- Identify diagnoses and formulate lists of current and past medical conditions
- Communicate findings with members' PCPs
