Elevance Health is seeking a highly skilled Family Nurse Practitioner to join our team at Paragon Infusion Centers. As a key member of our clinical team, you will provide exceptional patient care and support our mission to improve lives and communities.
Key Responsibilities- Administer ordered therapies according to prescriber orders, facility protocols, and company policies.
- Complete admission processes as directed by the Center Director.
- Perform physical assessments on patients as warranted and for new consults.
- Document all pertinent data in patient medical records.
- Report significant changes or observations to the Center Director and follow up on identified problems.
- Communicate effectively with IV team members on patient status and observations.
- Maintain patient confidentiality and adhere to HIPAA laws and regulations.
- Evaluate patient responses to therapy and document findings in medical records.
- Educate patients and/or family members regarding therapy plans.
- Participate in Quality Improvement programs.
- Ensure medication orders are complete, accurate, and up-to-date prior to treatment.
- Provide clinical coverage for nurse practitioners on PTO/LOA and support new market launches.
- May require extensive travel to worksites and other locations as necessary.
- Requires an MS in Nursing and a minimum of 2 years of nursing experience, or any combination of education and experience providing an equivalent background.
- Current, active, valid, and unrestricted NP license to practice as a healthcare professional within the scope of license in applicable states.
- Active, current, and valid Family Nurse Practitioner Certification.
- Multi-state licensure required if providing services in multiple states.
- Satisfactory completion of a Tuberculosis test and Hepatitis B vaccine or signed waiver.
- Minimum of 1 year healthcare experience with IVs strongly preferred.
- Minimum of 1 year IV experience strongly preferred.
Elevance Health operates in a Hybrid Workforce Strategy. Unless specified as primarily virtual by the hiring manager, associates are required to work at an Elevance Health location at least once per week, and potentially several times per week. Specific requirements and expectations for time onsite will be discussed as part of the hiring process. Candidates must reside within 50 miles or 1-hour commute each way of a relevant Elevance Health location.
