Primary Care Physician

Job Summary:
We are seeking a highly skilled Primary Care Physician to join our team at a Community Health Organization in Connecticut. As a Primary Care Physician, you will provide comprehensive health management, preventive care, and coordination of care to patients.
Key Responsibilities:
- Provide ongoing and holistic care to patients, managing a wide range of health issues from acute illnesses to chronic conditions.
- Focus on preventive medicine, conducting routine check-ups, vaccinations, and screenings to promote wellness and detect health issues early.
- Act as the central point of contact for patients, coordinating care between specialists and other healthcare providers to ensure continuity and efficiency.
- Educate patients about their health conditions, treatment options, and lifestyle choices, offering counseling to support behavior changes that promote better health outcomes.
Requirements:
- MD or DO degree
- Board Eligible or Certified in Family Medicine, Primary Care, Internal Medicine, or Adult Medicine
