Job Title: In-Home Care RN

Family Tree Private Care is a leading provider of concierge-level advisory services to older adults, those living with disabilities, and their families. We are seeking a compassionate and skilled Registered Nurse to join our team of dedicated healthcare professionals.

Job Summary:

We are looking for a Registered Nurse to provide high-quality, patient-centered care to our clients in the comfort of their own homes. As an In-Home Care RN, you will be responsible for assessing and charting patient observations, completing evaluation tasks, and educating caregivers and family members on aftercare and ongoing care.

Responsibilities:
  • Travel to assigned patient's homes and manage their care plan according to physician's instructions.
  • Assess and chart observations of the patient's condition at each visit.
  • Complete evaluation tasks, including reviewing medication and vital signs.
  • Dress or redress wounds and assess the healing progress.
  • Educating caregivers and family on the aftercare or ongoing care of the patient.
  • Keep the patient comfortable through palliative care.
  • Recommend tools or devices that may improve the quality of life for the patient.
Requirements:
  • Strong interpersonal skills that allow them to get to know their patients, build personal rapport, and establish trust to provide high-quality care while increasing patients' enjoyment and comfort.
  • Initiative so they can make independent decisions when required and accept direction and requests from patients.
  • The ability to follow instructions but also act independently if necessary.
  • Excellent observational and problem-solving skills.
  • Ability to act swiftly in emergencies and work efficiently.
  • Flexibility and patience to work in a variety of living environments.
  • Knowledge of clinical skills including changing bandages, giving tube feedings, administering shots, and setting up and monitoring IVs.
Requirements and Qualifications:
  • Relevant license and certification.
  • A valid driver's license and reliable transportation.
  • Strong candidates have a minimum of 1 year of experience working in the home care industry (or related industry - hospice, home health, hospital, etc.).
