Available Shifts:
All shifts require weekend availability.
12:30 PM - 8:30 PM - Saturday to Wednesday (Thursday & Friday off)
7:30 AM - 3:30 PM - Tuesday to Saturday (Sunday & Monday off)
2:30 PM - 10:30 PM - Thursday to Monday (Tuesday & Wednesday off)

Hiring Bonus: $1,000

Role Overview:

As a Personal Care Assistant, you will support clients in achieving their individualized goals and participate in community-based activities, emphasizing client choice, health, and safety. You will deliver compassionate care through various tasks, including personal care and assistance with daily living activities. This role requires adherence to policies and procedures established by Evergreen Supported Living, as well as compliance with state regulations.

Key Responsibilities:

  1. Build trusting relationships with clients to ensure effective support and service delivery.
  2. Assist clients in fostering independence and learning essential life skills.
  3. Support clients with personal care, household management, meal preparation, shopping, and health maintenance.
  4. Implement Positive Behavioral Support strategies as outlined in individual plans.
  5. Engage positively with family members, case managers, and other stakeholders.
  6. Provide transportation for clients as needed.
  7. Utilize Individualized Support Plans to help clients achieve their goals.
  8. Maintain accurate documentation of client progress and care.
  9. Participate in staff meetings and training sessions as required.
  10. Ensure a clean and safe environment for clients and staff.
  11. Assist clients in accessing healthcare services and managing their health.
  12. Support clients with personal hygiene and grooming tasks.
  13. Perform additional duties as assigned.

Qualifications:

  • Must be at least 18 years old.
  • High school diploma or equivalent.
  • Certification in CPR/First Aid and other relevant training.
  • Valid state driver's license with a clean driving record.
  • Ability to pass a background check.
  • Physical capability to assist clients as needed.

Preferred Qualifications:

  • Experience working with individuals with developmental disabilities.
  • Strong communication and teamwork skills.

COVID-19 Considerations:

  • Proof of COVID-19 vaccination required upon hire.
  • Personal protective equipment and sanitization measures in place.

Reports To: Program Manager

Compensation: Hourly wage commensurate with experience.
