Role Summary
The Clinical Case Manager is responsible for managing the service planning and transition processes for youth entering and exiting the program. This role includes leading the Treatment Team, creating Treatment Plans, assessing youth needs, and ensuring the timely provision of necessary program services in alignment with the Personal Growth Model.
Key Responsibilities
- Conduct intake assessments for youth upon program entry and manage re-assessments according to established guidelines. Ensure all youths undergo academic, vocational, and behavioral evaluations within the required timeframe, utilizing this data to inform individual service/care plans.
- Implement Evidence-Based Treatment models as per program requirements, ensuring adherence to fidelity standards.
- Facilitate monthly youth progress meetings with the treatment team, or more frequently as necessary.
- Perform home visits with youth's families in compliance with program and state regulations, aiding families in understanding the impacts of group living and family separation.
- Organize transitional meetings prior to the youth's release, assisting in the formulation of a Transition Plan in accordance with contractual deadlines.
- Manage administrative duties related to the position, including:
- Preparation and distribution of comprehensive monthly reports to parents/guardians, judges, and probation officers within the required timeframe following MDT meetings.
- Ensure advisors provide weekly behavioral updates and track goal progress.
- Conduct monthly audits to verify the accuracy of recorded information.
- Maintain detailed case management and service notes.
- Compile and manage youth exit interview data.
- Coordinate off-site appointments and transportation for youth.
- Represent the program at court hearings, meetings, and legal proceedings involving youth.
- Support the establishment of positive youth behavior and discipline, intervening in conflicts as necessary.
- Lead or participate in interdisciplinary meetings focused on positive reinforcement and care reviews.
- Act as a liaison between internal and external stakeholders.
- Attend required training and professional development sessions, maintaining relevant certifications and licenses.
- Assist with special projects and additional responsibilities as assigned.
Qualifications
- Bachelor's Degree in Psychology, Social Work, Counseling, or a related field with a focus on human behavior and development, counseling techniques, and therapy.
- A Master's Degree in Psychology, Social Work, or a related counseling field is preferred; license eligibility is highly desirable.
- Two years of experience working with individuals facing serious emotional disturbances or substance abuse issues is preferred.
- Experience providing counseling and training to families in home settings is highly desirable.
