LPN Homecare Job Description

We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality patient care in the comfort of their own homes. As a LPN, you will be responsible for assessing, planning, implementing, and evaluating patient care to ensure optimal health outcomes.

Key Responsibilities:

  • Utilize the nursing process to assess patient needs and develop individualized care plans.
  • Provide skilled nursing care and services in accordance with prescribed orders.
  • Collect and analyze data to identify potential nursing diagnoses and develop interventions to address them.

Requirements:

  • Current licensure as an LPN/LVN in the state of practice.
  • Current TB or Chest X-Ray.
  • Current BLS card.
  • One year prior professional nursing experience preferred.
