We are seeking a highly skilled and compassionate Licensed Social Worker to join our team at Outfield Healthcare Partners. As a Geriatric Social Worker, you will play a vital role in fostering a climate that enables residents to maximize their individuality, independence, and dignity.
Key Responsibilities
- Conduct thorough social histories and psychosocial assessments to identify the unique needs of each resident.
- Develop and implement individualized care plans that address the physical, mental, and psychosocial needs of residents.
- Maintain accurate and timely documentation of resident progress and care plans, ensuring compliance with federal, state, and local regulations.
- Collaborate with interdisciplinary teams to ensure seamless care delivery and resident satisfaction.
- Support residents' rights to self-determination and autonomy, while also providing education and guidance on therapeutic alternatives.
Requirements
- Certified, licensed, or registered in the state of practice, required.
- Bachelor's Degree in Social Work or a related field, including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology.
- One year of supervised social work experience in a healthcare setting working directly with geriatric individuals.
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
