The University of Iowa Health Care, Department of Pharmaceutical Care is seeking a part-time (60%) Pharmaceutical Care Specialist to join our Internal Medicine team.

The pharmaceutical care specialist is accountable for all facets of medication management in the adult acute care demographic.

This role encompasses care in the intermediate and internal medicine sectors, including but not limited to, internal medicine, surgical services, family medicine, and orthopedic populations.

Furthermore, this position may involve training in other inpatient clinical areas as staffing requirements dictate. The position entails rotating shifts, which may include weekdays, weekends, and evenings.

Pharmaceutical care specialists at UIHC engage in all dimensions of patient care, such as therapeutic drug monitoring, medication reconciliation, medication education, and mentoring pharmacy learners.

This role is pivotal in supporting the highest standards of safe and effective patient care through meticulous clinical review and verification of medication orders.

Moreover, all members of the pharmacy team collaborate daily with members of the interdisciplinary team, including licensed independent practitioners, medical students and residents, nurses, social workers, and care coordinators.


Key Responsibilities:
Provides Therapeutic Drug Monitoring (TDM)
  • Evaluate medication orders to ensure accuracy and appropriateness
  • Adhere to policies and procedures relevant to the patient care area
  • Ensure pharmacokinetic assessment of drug dosing on suitable patients
  • Communicate with prescribers regarding patient medication needs
  • Identify and assess adverse drug reactions in patients
  • Provide drug information to other healthcare professionals
  • Confirm/clarify patient drug allergies noted in medical records when necessary
  • Complete medication histories for all patients and reconcile home medications against the admission medications
  • Review and reconcile all medications upon patient transfer between clinical services
  • Review daily therapeutic monitoring information for all patients served by the patient care unit (and other patient care units covered by the pharmacist's healthcare team)
  • Identify high-risk medication therapy by defined admission criteria and review as appropriate
  • Complete required competency exams as mandated by the department

Documentation of Patient Care Activities:
  • Identify, evaluate, and take measures to prevent adverse drug reactions in all patients
  • Update patient allergies and patient-specific height and weight information as needed
  • Review and ensure accurate medication histories are entered into the medical record
  • Provide and document medication counseling to patients for targeted populations
  • Document therapeutic drug monitoring assessments and recommendations following departmental policies and procedures

Coordination of Patient Communications/Interactions:
  • Perform medication history and reconcile home medications with inpatient orders
  • Discuss drug allergy history
  • Conduct patient education sessions (where appropriate)
  • For targeted high-risk populations, provide medication counseling at discharge

Leadership in Patient Care Initiatives:
  • Coordinate medication orders written during assigned hours of duty
  • Follow up on medication system issues
  • Implement, through interventions and education, protocol antibiotic and DUE initiatives
  • Participate in multidisciplinary initiatives
  • Serve on intra- and inter-departmental committees as required

Educational Services:
  • Act as a preceptor for pharmacy students on clinical rotations sponsored by the UIHC Department of Pharmaceutical Care
  • Deliver in-service educational programs to staff nurses, physicians, and other members of the healthcare team
  • Participate in and present at other educational programs sponsored by the Department of Pharmaceutical Care, the College of Pharmacy, other UIHC departments, and state and national organizations
  • Coordinate/conduct patient medication education classes (where appropriate)

Education Requirements:
Pharm. D. (Doctor of Pharmacy) degree or Bachelor of Science in Pharmacy
Current licensure by the Iowa Board of Pharmacy Examiners is required. Includes those eligible for obtaining this licensure.

Experience Requirements:
One (1) year of relevant pharmacy experience is required. Applicable experience includes the time spent completing an ASHP-accredited pharmacy residency.
Excellent written and verbal communication skills
Job-related experience with and/or commitment to service excellence
Professional experience working effectively with individuals from diverse backgrounds and perspectives.

Desired Qualifications:
Experience in a hospital pharmacy setting is highly desired
Basic Life Support (BLS) certification or eligibility for completing BLS certification
Satisfactory completion of an ASHP-accredited PGY1 residency in pharmacy
Satisfactory completion of an ASHP-accredited PGY2 residency in a specific field
Clinical experience in providing pharmaceutical care to patients in internal medicine areas
Board certification in Pharmacotherapy or another specialized field
