Job Title: Regional Case Manager

We are seeking a highly skilled and compassionate Regional Case Manager to join our team at Community Health Plan of Washington. As a Regional Case Manager, you will play a critical role in coordinating care management services for our members with complex medical and behavioral needs.

Key Responsibilities:
  • Coordinate case management services for members with short-term, long-term, stable, unstable, and predictable courses of illness.
  • Provide case management services for members with highly complex medical/behavioral and social conditions, requiring advocacy and coordination.
  • Represent Community Health Plan of Washington at regional meetings, presenting and speaking on care management and care coordination topics.
  • Collaborate with the Regional Manager to provide external representation at community and program-based meetings and initiatives.
Requirements:
  • Bachelor's degree in nursing or a master's degree in social work and/or a related behavioral health field.
  • Current, unrestricted license in the state of Washington as a registered nurse (RN), social worker (LSWAA, LSWAIC), mental health counselor (LMHC), mental health professional (LMHP), or marriage and family therapist (LMFT).
  • Minimum of one (1) year of case management experience.
  • Minimum of two (2) years of experience in using care management workflow systems, NCQA standards, and experience meeting with and presenting to organizations.
Preferred Qualifications:
  • Case management certification.
  • Bilingual.
  • Experience in managed care (Medicaid/HCA).
  • Experience in using Care Management software applications.
  • Knowledge of and experience with community resources.
