As a Home Health Care Professional, you will be responsible for delivering comprehensive care services, including medication administration, wound care, and treatments. You will work closely with clients and their families to develop personalized care plans that meet their unique needs.

Responsibilities
  • Deliver high-quality care services to clients in their homes
  • Develop and implement personalized care plans in collaboration with clients and their families
  • Administer medications, provide wound care, and perform treatments as prescribed by physicians
  • Monitor clients' conditions and report changes to Clinical or Client Services Manager
  • Follow up with and execute doctors' orders
  • Perform client assessments as necessary
  • Case management and coordination

Qualifications for a Nurse Specialist:

  • A current license as a Registered Nurse in New Hampshire
  • A minimum of one year of recent, verifiable clinical (medical/surgical) experience
  • Prior home care experience strongly preferred, but not required
  • Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
  • Ability to work independently and manage time effectively
  • Strong interpersonal skills
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred
