Key Responsibilities- Coordinate patient care plans under the supervision of a Registered Nurse
- Assess, plan, and implement patient care in accordance with professional nursing standards
- Collaborate with other healthcare team members to ensure seamless care delivery
- Work within the scope of practice for Licensed Practical Nurses in New York State
To be successful in this role, you will need:
- Current LPN licensure in New York State
- Experience working in home care or a similar setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
