About the Role

We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our Hospice Care team at Sangre de Cristo Community Care. As a Hospice CNA, you will play a vital role in providing comfort and support to our patients and their loved ones during their most vulnerable moments.

Responsibilities
  • Provide holistic care to hospice patients in their homes or residential facilities, focusing on their physical, emotional, and spiritual well-being.
  • Assist patients with activities of daily living, including bathing, grooming, and toileting, with empathy and patience.
  • Monitor and report changes in patient condition to the interdisciplinary team, ensuring coordinated care and support.
  • Offer emotional support and companionship to patients and their families, creating a safe and comforting environment.
  • Collaborate with nurses, physicians, social workers, and other team members to ensure seamless care and support.
