The Community Health Worker III (CHW-III) is a key member of our team at Providence California Regional Services, responsible for helping community residents and Medical Center patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.

Providence Health & Services is a not-for-profit Catholic health care ministry committed to providing high-quality, patient-centered care to those in need. Our caregivers are dedicated to delivering whole-person care, built on understanding, commitment, and mutual respect.
Responsibilities:
- Assist community residents and Medical Center patients and their families in accessing community services and resources.
- Help individuals and families adopt healthy behaviors and navigate the healthcare system.
- Collaborate with healthcare teams to provide comprehensive care and support.
- Develop and maintain relationships with community partners and stakeholders.
- Provide education and outreach to promote health and wellness.
Requirements:
- Education to meet certification, license, or registration requirements.
- 1 year of experience in a combination Community Health Worker I and II position, or 2 years of related community outreach, enrollment, education, or other community health administrative experience, or a Bachelor's degree in lieu of experience.
- Valid driver's license and auto insurance (upon request).
