Schedule Information: Clinics operate from 7am-7pm Monday to Thursday, and 7am-5:30pm on Friday. Availability for a minimum of 4 shifts per month is required, with hours and days varying based on needs. This role does not include weekends or holidays and focuses exclusively on Outpatient Therapy Services at various clinic locations.

Your Responsibilities:

Assess individuals referred for therapy services. Formulate a therapy diagnosis, prognosis, and treatment plan. Continuously evaluate the effectiveness of treatment. Collaborate with and provide guidance to others involved in care delivery. Oversee the work of therapy assistants and aides.

Qualifications:
  • Bachelor's Degree in Physical Therapy or higher.
  • Current State of WI Physical Therapist licensure.
  • Current BLS (CPR) certification or ability to obtain upon hire.
  • Preferred: At least 1 year of relevant therapy experience.
