Position Title: Consumer Services Coordinator Specialist - Floater - Bilingual Spanish

ROLE SUMMARY: This specialist role encompasses a wide range of responsibilities that facilitate effective case management. The individual will provide essential information, advocacy, and service coordination for consumers and their families within assigned caseloads.

SUPERVISION: The position operates under the general oversight of the Branch Supervisor at branch locations and the Consumer Services Supervisor at the main office. In the absence of a Branch Supervisor or Consumer Services Manager, the CSC Specialist - Floater will report directly to a Director.

KEY RESPONSIBILITIES: The following duties exemplify the types of tasks that may be undertaken. Additional responsibilities may be assigned as necessary:

  1. Engage in communication with consumers, families, service providers, and external agencies regarding service eligibility, regional center standards, and problem resolution. Utilize various communication methods, including phone and email.
  2. Facilitate individual program planning, including annual and quarterly reviews, in collaboration with consumers, families, and service providers, while completing necessary documentation.
  3. Conduct individual family service planning, including initial assessments and periodic reviews, with consumers and their guardians.
  4. Generate referrals and manage Purchase of Service Authorizations, including changes and terminations as required.
  5. Provide advocacy support as needed.
  6. Disseminate information about available resources and services to consumers, families, and care providers.
  7. Document all case management activities in accordance with established guidelines.
  8. Maintain case management records and ensure accurate documentation.
  9. Oversee the Community Resource Database.
  10. Review incoming case transfers, ensuring proper evaluation of funding authorizations.
  11. Assess outgoing case transfers to confirm appropriate funding terminations.
  12. Review Notices of Actions.
  13. Assist in training and mentoring new staff members.
  14. Participate in diversity and inclusion initiatives, community outreach, and special projects.
  15. Perform additional related duties as assigned, including providing backup support as necessary.

EMPLOYMENT REQUIREMENTS:

Knowledge: Understanding of developmental disabilities and social service provision.

Skills: Proficient in assessment, program planning, customer service, and strong organizational abilities. Excellent verbal and written communication skills are essential, along with the capability to convey complex information effectively. Must demonstrate proficiency in computer usage and work well with diverse populations.

Qualifications: A Bachelor’s Degree in social work, psychology, human development, sociology, public health nursing, or a related field is required, along with two years of preferred experience in a regional center as a service coordinator. A Master’s Degree in a related field is preferred.

Additional Requirements: Frequent communication via phone and attendance at out-of-office meetings are necessary. A valid driver's license and reliable transportation are required. This position is classified as non-exempt.
