As a Hospice Registered Nurse (RN) Case Manager, your responsibilities will include:
- Reporting directly to the Administrator, Executive Director, or Patient Care Manager Senior, while maintaining primary oversight and professional management of each patient.
- Serving as the main point of contact between the physician, patient/family, and hospice team.
- Delivering direct patient care services to hospice patients and providing supportive care to their families, ensuring a manageable caseload and fostering a trusting relationship.
- Empowering caregivers by educating them on how to care for their loved ones effectively.
- Collaborating with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff, to ensure comfort, peace, and dignity for our patients.
- Assessing patient and family needs and assisting in the development and implementation of individualized care plans.
- Maintaining accurate clinical records for all assigned patients.
- Participating in quality assessment and performance improvement initiatives within the organization.
Candidate Qualifications:
- A compassionate heart, demonstrating kindness and empathy towards patients and their families.
- Comfortable providing care in various settings as designated by the patient.
- Ability to remain flexible and composed while supporting others during crises.
- Current RN license in the state of employment (or eligibility to obtain).
- A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatrics, or oncology is strongly preferred.
- Additional preferred experience includes roles such as hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatrics, admissions, wound care, telemetry, or travel nursing.
- Possession of a valid driver's license and automobile insurance.
- Current CPR certification.
