The ideal candidate will implement patient plans of care, adhere to regulations, and provide optimal patient care.
Key Responsibilities:
- Deliver pharmaceutical care through patient-centered clinical service, medication information, education, and distribution.
- Implement patient plans of care based on diagnosis in a timely manner.
- Adhere to regulations, codes, and procedures for optimal patient care.
- Collaborate with patients, families, and healthcare providers to ensure safe, effective, and cost-efficient medication therapy.
Requirements:
- Doctorate in Pharmacy
- PHARMD-Licensed Pharmacist or PHARMR-Registered Pharmacist
- 1 year of relevant experience
Work Environment:
- Evening shifts
- Part-time schedule
- Variable days of the week
- Rotating weekends
