Job Summary:
As a Nurse Navigator at LifeBridge Health, you will play a vital role in identifying and managing high-risk patient populations, coordinating their care, and providing safe, age-appropriate patient care. This position requires a collaborative team approach, working closely with healthcare professionals to ensure seamless patient care.
Key Responsibilities:
- Develop and implement patient care plans to address the unique needs of high-risk populations
- Collaborate with healthcare teams to coordinate patient care and ensure timely interventions
- Provide education and support to patients and their families to promote health literacy and self-management
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
Requirements:
- Basic professional knowledge equivalent to a Bachelor's degree in a specialized field
- 3-5 years of experience in oncology nursing, with a focus on patient navigation and care coordination
- BSN degree mandatory, with OCN certification
- 4-5 years of oncology nursing experience, with a preference for outpatient settings
- Maryland Registered Nurse License and American Heart Association CPR Certification
