We are seeking a skilled and compassionate Registered Nurse to join our Senior Community Care team as a Field Clinician. In this role, you will provide high-quality care to our highest-risk health plan members in a facility setting.

Key Responsibilities
  • Ensure accurate and complete documentation of patient care
  • Collaborate with the interdisciplinary care team to develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Assist with benefit determination and coordination of care services
  • Participate in facility-based meetings and contribute to quality improvement initiatives
Requirements
  • Associate degree or higher in Nursing (RN)
  • Active and unrestricted license in the state of New Jersey
  • Certified in Basic Life Support
  • Ability to complete physical requirements, including lifting a 30-pound bag
  • Access to reliable transportation
Preferred Qualifications
  • 1+ years of clinical experience in an acute care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
