**Role Responsibilities:**
The RN Case Manager engages in advanced nursing practices, focusing on the assessment, care, and coordinated treatment of individuals at the state-supported living center.

This role involves overseeing nursing care delivery and monitoring the health status of assigned individuals, ensuring their healthcare needs are met effectively.

As part of an interdisciplinary team (IDT), you will collaborate with various professionals to maintain clear communication and provide holistic care.

Key tasks include conducting physical assessments, documenting healthcare requirements, developing care plans, and evaluating the effectiveness of nursing interventions.

You will also proactively address healthcare outcomes, review documentation for any issues, and create comprehensive nursing care plans that guide necessary interventions.

**Essential Functions:**
- Regular attendance and adherence to assigned shifts, including potential on-call duties.
- Monitor the health status of individuals and conduct rounds to gather insights from support staff.
- Utilize nursing skills to identify and address emerging health issues, ensuring proactive care is delivered.

- Participate actively in the IDT, providing nursing expertise and advocating for the needs of individuals.

- Maintain accurate documentation and complete required assessments in accordance with established protocols.

- Attend medical appointments with individuals to ensure continuity of care and effective communication.

- Engage in ongoing training and education to uphold nursing best practices and compliance with safety standards.

- Foster professional relationships with individuals, colleagues, and stakeholders, contributing to a collaborative work environment.

- Participate in quality assurance initiatives to enhance nursing policies and procedures.

**Qualifications:**
- In-depth knowledge of nursing practices, healthcare regulations, and treatment protocols.
