We are seeking a skilled Licensed Practical Nurse to join our team at Watertown Health Care Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.

Job Summary

The Licensed Practical Nurse will be responsible for supervising Certified Nurses Aides, collecting patient care data, and implementing patient care plans. You will also contribute to the development of discharge plans and participate in the orientation and education of personnel and students.

Key Responsibilities
  • Supervise Certified Nurses Aides under the direction of the Registered Nurse
  • Collect patient care data and monitor patient conditions and family dynamics
  • Contribute to the development of the plan of care and discharge plan for assigned patients
  • Implement patient care utilizing care plans, delegated medical orders, and collaborate with health team members as necessary
  • Assist in evaluating outcomes of patient care planning and teaching and in revising the plan according to changing needs of the patient
  • Participate in the orientation and education of personnel and students
  • Document the delivery of health care and nursing process in accordance with specific unit standards and hospital nursing policy
Requirements

To be successful in this role, you will need to have a Graduate from an accredited school of licensed vocational nursing or licensed or licensed practical nursing with current state license in good standing. Two or more years of acute care or rehabilitation experience as a LPN is preferred.
